If you are very tired then you need to think extra hard about safety, as lapses in concentration can be dangerous.
For example, before you get in the car it is sensible to stop and think, am I too tired to drive? If you aren’t sure then it probably isn’t safe. If you can’t avoid the journey then think about other ways of doing it e.g. ask a neighbour to drive you or take public transport. Or can you put it off until later and try to find a way to rest or take a nap in the meantime?
